    Mike,

The first place to start, IMO, is with your State Chemist/oversight office that regulates your state. Each state has it's own specific documentation required on the application logs. Then create a template based off the state requirements and your needs. The state may even have a sample form that you can tailor to fit your needs.

In Florida, we aren't required to document any application, except Restricted Use. However, it is highly advised that all applications are recorded, and if they are, the state has the right to inspect them. So, I created this form in Excel. It's half of an 8 1/2" x 11" page of paper. I had a printing company print them out in pads of 200 per pad. We write all fertilizer and pesticide applications on this and hand them to the spray tech's. When it is completed, it gets hole punched and put in a binder.

For Restricted Use, we use the form that the Florida Department of Ag suggests, as attached.

Only thing I would change on the next set of printing is to print it onto NCR paper so that one copy goes with the spray tech, one to the Assistant, and one to the Superintendent. Basically, three copies of the records.

Have used sheets that we print off, now have spreadsheets created in google sheets that house all of the necessary information from rate, weather, applicator, etc to document everything. Use individual sheets for tees, greens/app, fairways, and rough; 1 log for liquid/pesticide apps and one for granular apps.

    Andy,

Love the idea. What company made them for you? I'm interested in looking into implementing something very similar. Thanks.

I created it and had a printer print. Office Depot could even do it for you if you provide the template. From what I remember, it was around $250 for around 5,000 sheets.

Shoot me an email and I'll send you the template. You can doctor it up anyway you want.
