In Florida, we aren't required to document any application, except Restricted Use. However, it is highly advised that all applications are recorded, and if they are, the state has the right to inspect them. So, I created this form in Excel. It's half of an 8 1/2" x 11" page of paper. I had a printing company print them out in pads of 200 per pad. We write all fertilizer and pesticide applications on this and hand them to the spray tech's. When it is completed, it gets hole punched and put in a binder.

For Restricted Use, we use the form that the Florida Department of Ag suggests, as attached.
Only thing I would change on the next set of printing is to print it onto NCR paper so that one copy goes with the spray tech, one to the Assistant, and one to the Superintendent. Basically, three copies of the records.
