   Wondering if anyone has an example of a survey sent out to your membership covering multiple areas of the facility. Thanks in advance for any help.

   I do not have specific questions but just think of what your members talk about the most and get a general opinion on the features of your club. Most of all keep it simple and easy.
   
   If most of your membership knows how to use a computer I would suggest using Survey Monkey as a easy to use easy to take easy to see the results tool.
